She's never flaked on me before, we rescheduled one date earlier but I get a weird sense I'm being setup for a flake.

Any good prevention? ignore? Make other dates? smash?

As a general rule, unless you know the girl to be reliable based on past behaviour you should have another girl, or at the very least another activity scheduled for the same time. You can't discern reliability in 4 dates.

That way if she flakes you will always have something or someone else to keep you occupied.

So 4 dates... have you had sex with her yet? Is she initiating texts to ask how you are throughout the week? This could be a case of being super busy or luke warm attention. Don't reschedule then. If she's got even slight high interest and you disappear for a few days, she'll text you.

Yeah pretty much, at first at lesst...after a while they may drop you if they don't feel like you are really into them other than the sex however...
