Now, I'm wondering, do women really know what they're doing or are they simply acting these things out of pure emotion or intuition maybe?
It's hard to put it as simple as yes or no. Women will do some things totally aware they're doing it and some things instinctively (a sh!t test is normally something she's unaware of but sometimes will be conscious of it).
I wouldn't get caught up in this too much; it's better to just let the actions determine her place in your life. But let's see....
In the example you link - the cold sore excuse is a clear indication of low interest. And that’s the most important thing! If it was the truth - why would you advertise it to a guy you were interested in? If it's not the truth - why would you pick such a disgusting excuse? (other than not caring)
In my opinion - most women are cognitive dissonance machines! Meaning - they don't really know why they're making decisions a lot of the time; they simply make the decision then figure out why through some form of (il)logic (this is why it's always better to judge on action instead of words). Same thing most PEOPLE do when shopping.
Just last week - a girl calls me at midnight. I answer, I'm expecting her to want to ask to come over for a late night booty call. When I ask her if she's coming over, she hesitates, then says "I don't know how to get there from here." I remind her of the existence of GPS. 3-4 other excuses follow (i have no idea why she actually called me.) She made the decision on not coming over to my house off of some emotional madness in her brain. Then (il)logically started coming up with reasons - aka: excuses.
I don't believe that's something most women do on a conscious level.
Sometimes it is done with total consciousness though. I just don't think there's any way to know unless it's painfully obvious.

Or perhaps, their strategic thinking about the matters of getting attention, or getting the guy they like to chase them, something that was developed over years of practice and trial-and-error?
Most tests are done without consicousness of them. But, she could also be trying things out to get you to chase.... Like:
She could have read it in cosmo ("the secret to finding out just how much the man you're talking to really likes you with these 10 questions" etc.)
She could have got "advice" from one of her female friends or beta orbitors (most men underestimate the amount of "game sharing" women do among each other. I've even overheard mothers telling daughters how to manipulate men into doing things like: take out the trash.)
She could have read that stupid "he's just not into you" book. Which if she ever reads while talking to one of us - she'll instantly think we aren't into her at all.
Or she could be using her scrambled egg head to come up with her own ideas.  (this will normally be apparent by the simpleness of it; followed by a sly grin on her face when you call her out on it.)
Note: When messing with younger girls (18-21) - I do notice very clear signs of actively trying to manipulate to get their way/play games to make chase, etc. I dated a very hot 18 year old briefly last year. And her attempts to get her way/make me chase were comical and definitely happening on a conscious level... I used to tell her "Step your game up girl. You suck at this" on a regular bases. And that stupid ('he caught me') grin would find it's way to her face...
She got the last laugh though....
