Nobody outside of his close family and friends knows for sure, but there's been a lot of speculation it was due to a heart attack. However given that he had a number of health issues, it could've been any of those or something related to or triggered by them.

Heart attack is certainly a probable cause for a diabetic 59 year old black male that admitted to years of horrific eating habits. Diabetes is a huge risk factor for cardiovascular disease. Diabetes and pre-diabetes / insulin resistance are usually found undiagnosed in heart attack victims that have otherwise normal cholesterol levels.
