She smiled politely. Nodded. And never texted you back.

6. Spending lots of time on phone (texting)

If she is about to branch swing then the most definitive sign will be provoking you into a confrontation and argument thus giving her justification to end the relationship.

Her flakey behaviour suggests that she has already begun this process.

Just dump her and dont give her a reason. She knows what the reason is all too well. Pull the rug and watch her stupid reaction and laugh.
