basically, there are two effects: (1) intramuscular water retention; (b) creatine itself is energy for lifting.
the first effect is believed to aid muscle development. the temporary water gains that make you look fuller will disappear after you stop taking it, but there are at least two reasons to believe it can help. i've never used the stuff, but this appears to be the consensus on how it could be beneficial.
if you're gauging your progress based on ever-increasing weights (strength), then you shouldn't need to worry too much one way or the other about size.
