Serious help needed for future LTR's

I really need some help or advice on LTR's for when I get in a new one.

I seem to do OK meeting girls, and dating them for a while and sleeping with them. I even get them to the exclusive stage

I dated one girl for close to 3 years, but then with time it fell apart.

But the vast majority of my relationships have failed before the 6 month mark.

It is depressing.

What advice do you have to keep it going?

Just an FYI, most relationships that have a high probability of failure will fail by the six month mark. Don't take it personally, it happens to everyone.
